Membership forms are available in the OSHS office and at any PTSA sponsored event. Annual membership costs $5 per individual. Anyone can join. Contact us at OSHSPTSA.MEMBERSHIP@cox.net or download this form, print and return it to the PTSA box in the school office or mail it to OSHS ATTN:OSHS-PTSA at the address to the left.
